Service Needs Coordinator (Full Time-Contract) – Douglas Crossing Retirement Community

Candidates are invited to apply for the Full Time-Contract Day Shift Service Needs Coordinator position at Douglas Crossing a retirement community, located in Uxbridge, Ontario. The Service Needs Coordinator will report to the Director of Wellness in overseeing clinical activities within the community designed to promote the well being of the residents.

As a The Service Needs Coordinator your typical day will involve the following;

  • Overseeing the clinical activities within the community designed to promote the wellbeing of our residents.
  • Collaborate with physicians, other health care professionals and family members to ensure resident needs are assessed; plan of care developed and determine corresponding care fees and team member requirements as applicable
  • Monitoring all resident incidents and ensuring reoccurrence is prevented by continuously evaluating resident needs and adjusting plan of care accordingly
  • Monitoring quality of resident care provided by nursing and related service providers is top quality and consistent with resident needs
  • Responsible for ensuring all staff and residents are following outbreak protocols
  • Developing professional and collaborative relationships with hospital team members and interviewing prospective residents and conducting assessments to determine suitability for admission.

To match our Qualifications, you will possess

  • Must be a Registered Practical Nurse (RPN) or a Registered Nurse (RN) Registration must be with the relevant provincial licensing body
  • 2-3 year Retirement Home experience
  • 1-2 years Clinical experience
  • Supervisory experience
  • High standards for infection control policies.
  • Strong attention to detail
  • Experience with older adults is an asset
  • Superior communication skills with fluency in English
  • Strong organizational skills and maturity
  • Ability to effectively listen and respond to the needs and concerns of our residents and escalate when appropriate
  • A clear criminal record
